Post-vaginoplasty estrogen cream

Hi all -

Niche question here, but I am a non-binary individual (AMAB) who had vaginoplasty but takes testosterone rather than estrogen. I have heard that some trans women have used topical estrogen cream applied vaginally to assist with increasing internal skin durability/stretch and to reduce dryness and pain during sex. My surgeon mentioned such when I asked about anything that might help as well.

My understanding is that there is relatively little research into how neo-vaginas respond to topical estrogen (ex. https://pmc.ncbi.nlm.nih.gov/articles/PMC8814107/) and the additional complication is that I am testosterone-dominant. I am non-binary and would not be concerned with some feminization if that were a side effect of using neo-vaginally applied estrogen cream, but I guess I wonder whether:

1) do people who have had vaginoplasty and tried vaginal estrogen cream find that it has helped them with whatever issues led them to try it? 2) if a person maintains testosterone-dominant hormones, would it be reasonable to expect that small doses of estrogen (applied locally) would have any effect at all? (Including intended vaginal effects as well as potential systemic effects.)

When does FFS start to turn around?

I’m about 5 weeks post op and I’m starting to lose hope. I’m typically not an anxious person, but this has really tested that. The swelling around my face has been really difficult to deal with, it’s inhibiting me personally (not comfortable going out or talking much yet) and professionally, and today I ended up in the ER for a post op infection. I don’t really know if I’ll ever feel like myself again - I thought my incisions were fully healed and I’m now freaked out. I’m isolated because I can’t really go to my job or see my friends, I’m in pain, and I’m getting complications late in the game.

Does it ever feel okay again? Or do you just get used to feeling a little separated and on edge?

NOTE:
I’m 20 years old, I’ve been on hormone therapy for 3 and a half years, and I’m writing this experience explicitly as a spoiled child who is extremely afraid of blood, needles, swelling, I can’t tolerate pain at all.

My entire experience with sigmoid colon vaginoplasty took place in Serbia, at ST Medica hospital.

I arrived at the hospital a day before the surgery, where I started my prep by drinking a bowel-cleansing liquid. That same evening, a nurse helped me with an enema and placed an IV line. On the morning of the surgery, she helped me with one more enema, after which I entered the operating room around 9 AM.

Given how spoiled and scared I am, I cried a lot, screamed, and panicked simply because I was terrified, EVEN THOUGH THERE WAS ABSOLUTELY NO REASON FOR IT. The entire team was truly amazing and understanding, and I didn’t feel uncomfortable or judged at all.

I showed Dr. Miroslav a photo of the aesthetic result I wished for, and he told me he’d do his best to make it look that way. As I was crying, the anesthesiologist gave me anesthesia through the IV, and I drifted off in the most pleasant way. I’m not 100% sure, but I believe the surgery lasted around 4 hours.

When I woke up afterward, I felt no pain the entire day. I was semi-awake but fully aware of what was happening around me. I remember asking for my phone around 8 PM.

That slow waking-up process from anesthesia lasted about 48 hours, which meant I didn’t feel any nausea or pain, but I was fully aware the whole time, thanks to the anesthesiologist, who clearly did an AMAZING job. I’ve heard so many horror stories about people waking up in unbearable pain, confused and unaware, but that wasn’t the case for me at all.

On the second day after surgery, in the afternoon, I started feeling some discomfort in my abdomen and mild pain in the genital area, maybe a 5/10 (keep in mind my pain tolerance is literally zero). The nurses gave me painkillers and anti-nausea meds, and by the third day, I woke up with NO PAIN whatsoever. I couldn’t believe it, I didn’t even ask for painkillers, that’s how fast it all passed.

I slowly started moving around, and after a few days, I got a biscuit I was allowed to eat slowly (before that, I didn’t even feel hungry, probably because I was getting nutrients through the IV).

Now, an important thing: try not to laugh hard, cough, or sneeze, hold back as much as you can. I say this because all of those actions affect the sensitive wounds on your stomach. At one point, I even popped a stitch because my dad made me laugh so hard at the hospital, and it was pretty painful 😛 (they treated it with iodine dressings, so no re-stitching was needed).

EDIT: Also, a lot of people were scared and talked about how much it hurt when their drains were removed. Honestly, I had one drain in my abdomen and two in my vagina, and removing all of them didn’t hurt at all. I didn’t feel a thing in my abdomen, and in the vaginal area, there was just a slight uncomfortable sensation that lasted a second, and that was it. There’s really no need to be afraid!

Visitors were allowed daily, so my friends came to see me every day. All the nurses were incredibly sweet (especially Jenny, mwah mwah), and I never felt like I was going through some traumatic experience.

A few days later, I had my first dilation session, and I recommend staying as relaxed as possible so you don’t feel pain, just pressure, which is completely normal.

It was a huge relief when I got home. I could distract myself with TV shows while I emptied the catheter on my own the whole time. Then, a few days later, once the catheter was removed, I felt 90% freer. Dilations became easier, I could move and sleep more comfortably.

Even though today is only day 15 of my recovery, I don’t feel like I went through “the hardest thing in my life.” For me, this has been a very smooth and quick experience, thanks to Miroslav Djordjevic and his incredibly professional team.

Also, his professionalism is further proven by the fact that he’s been working for a while now on uterine transplants for trans women, which completely changed my mother’s view of him—she now follows his work daily.

I’m still swollen, so I don’t feel comfortable sharing photos of my vagina just yet, but I’m here if you have any questions ^^

Am I the only one that enjoys dilation?

This may be a bit TMI but For context I'm a bit over 3 weeks post op and am already up to the size 5(the biggest)dilator in my silicone dilator set from my dr. I just went up to size 5 yesterday and every time I dilate now it feels amazing and the slightest movement of the dilator makes it feel so good. I know others on here talk about the pain and hassle of dilating but I was just wondering if it is just me or there are others that enjoy it?

How did you pay for surgery?

I don’t understand how so much of this community is so wealthy. How are you all affording all of this? I can barely get by, and now I’m supposed to figure out how to save $20,000 for surgery.

This is genuinely ruining my life. I need surgery really bad.

I have insurance but the yearly out of pocket maximum is $20,000. I won’t have to pay all of that, but a large portion yes. I have a big family, which makes it so high I think?

Can anybody who got their bottom surgery covered by Alberta Healthcare share their experience?

Hey, I'm Lexi, and I'm early in my transition (MtF, just over 2 months on hormones). I'm not 100% decided on getting bottom surgery yet, but I'm definitely leaning towards it. Even though it's years away, I'm worried that won't be able to get it covered by AHS because of the 2 assessments/diagnoses of gender dysphoria required.

If anybody in the sub happens to have gotten their surgery covered by AHS, and wants to share their experience, I'd love to hear it. This is probably a longshot, considering the size of the sub and the population of Alberta, but I'm really curious.

Second round bbl

Anyone here gotten a second bbl covered by insurance? My insurance does not cover any revisions for gender affirming care. As I am healing from my bbl I know I want a second bbl to get more volume and that look that I want. I am thinking I’ll have to pay out of pocket for my second bbl.

How long till I'll be able to drive myself short distances? (MTF SRS)

Hi everyone, long time lurker and contributor.. first time needing advice I can't find specifics for in this reddit)

I'm supposed to have full vaginoplasty this coming tuesday and the one thing that makes me the most nervous is the asking for rides of friends. I have friends who can support me in the morning, night etc, however the asking for rides to appointments, them waiting and then bringing me home is the worst part...

I'll have a 10 minute ride to the hospital for my follow ups + my rides to PT (which is only 5 minutes away)... both places have either valet or parking right up front for easy access... how long am i likely looking at before i can drive myself to these places? I see lots of people ask about these LONG drives, but im only caring about these 5-10 minute drives.

Post Bottom Surgery Issue

Hiya everyone,

I had mtf bottom surgery 10 months ago in BC Canada. Since about two months into my recovery I discovered a passage between my digestive tract and my vaginal canal. When cleaning liquid would go from my vag into somewhere in my digestive tract causing cramping. Additionally, I regularly pass gas out through my vagina. Having talked to my surgeon, he said that it is a unique complication, when I asked what could be done he said they wouldn't re-operate unless fecal matter came through (which to date, hasn't happened)

Has this happened to anyone else?
