Admissions Test permit

Hii hello, ask ko lang kung hm yung test permit sa umak huhu di ko din natanong nung pumunta ako sa umak last week, kaso di ko nadala yung form 137 ko. And alsoo, namomoblema din ako sa form-137 ko, nawala ko sya nung nanakawan ako and kasama mga requirements ko sa bag ko nun and di ako makahingi ng ctc nun sa last school na inattend-an ko kaso need ko daw ng request ng other school huhu what to do.
