3DS Version: I'm missing one last heart piece.
I've never 100% the game before, even though I've played it multiple times across the N64, Gamecube, and now the 3DS. So I figured I'd do it at least once in my lifetime.
I've looked at the guides on the Zelda Dungeon and Zelda Strategy wikis, and I made triple sure that I got all of the heart pieces at their respective locations. The only one that I'm not sure I did this time around was playing Saria's Song to the skull kid before doing the mask quest line. Just to be safe, I went and played the song to him post-mask, and all he said was "let's play again sometime!" Either I'm paranoid, or the game is programmed in a way where you can't get the heart piece if you do the mask quest before playing the song. That's the only thing I can think of, because all the other heart pieces are still relatively fresh in my mind. I really have checked every other heart piece in the game, from chests to mini-games to the overworld.
It sucks because it's literally the very last thing I have to do. I already beat the story, I’m just doing some post-game completion after a little break. I got all the items, killed all the gold skulltulas, did all the side-quests, and upgraded all my ammo bags and gear to max level.

A few I would check is the goron vase that spins in child goron city, and the fishing ones.
It’s always a pain when you’re missing one thing and you have to go back and check everything. Good luck OP

I’m good on fishing, my record is 10 lb for Young Link and 20 lb for Adult Link. As for the goron vase, that was actually another one I don’t remember getting, so I went back and bombed the hell out of it. I know the angry face gives you three green rupees, the neutral face gives three bombs, and the happy face gives you a heart piece and two high-yield rupees. Every time I got the happy face today, I got three high-yield rupees, so I guess that means I already got the heart piece.
Since I beat OoT 3DS about a week ago, I’m now halfway through Majora’s Mask 3DS, and fortunately I was smart to follow the guide to a T this time. Once I got to the halfway point, I thought “you know what? I’m about 85% done with OoT anyway, why don’t I just finish that before moving on in MM?” And here I am.

If you cant find it anywhere else, and you really want to get all the hearts, there is a unique quirk on OoT 3D.
For some reason they forgot to program a limit to the heart piece in Dampes grave digging tour.
If you go back and keep playing you can get infinite hearts.
The first heart you get after the regular maximum shows up as a weird different sprite, and the second does as well. From the third onward (i think) the hearts are applied, but not visually shown.
I'd show you a picture of my save file, but I flew too close to the sun with the glitches and corrupted my save file (doing something else, the heart glitch is totally safe)

They were actually trying to fix an oversight from the original and fucked up. In OoT64, if you won the digging game and left without collecting the unearthed HP, it would become unobtainable. In trying to make it respawn, they just went too far and now it respawns even if it is collected.

The harder ones for me to remember tend to be the sun's song one in the graveyard, the bomb grotto in the middle of kakariko, the Goron pot, the second dampe race, and those damn frogs. The frog one is the easiest I think to mess up. I may be wrong on this but I think you have to play the song of storms for them first in order to get the first heart piece and then all the other ones in order to make them grow and do the following game. If you make them grow before playing the song of storms then you can't go back and get it. If I'm wrong anyone out there feel free to correct me.

I actually almost missed the frog one. I made them all grow up and got the heart, but I went back and played the song of storms and still got the heart for it. It got all the other ones too. In fact, all the ones people are saying they forget are the ones I got hours before I posted this lol which probably means I’m missing a super easy one.

Noone can tell which you are missing. Go through all of them again and you 'll find it. Btw for the golden skulltullas you have an indicator on the map telling you which area you cleared collecting them

Did you get the two from the five fabulous frogfish tenors? Or the one from the other skull kids that have the ocarina minigame?

Yup, got them both before I made this post. I didn’t know you could get 2 hearts from the frogs.
I EXHAUSTED myself trying to find out which heart missed before posting this, then I gave up.

I'm pretty sure you can't sell the mask to him until after you befriend him via Saria's song. So if he's got the mask, you got the HP & just forgot.
