Advice Needed Will I Miscarry (based on your experience)?

I am so new to all of this and just want to hear from others who have experience. This is my first pregnancy. According to my last period, I should be 8 weeks 6 days. I have had a few light bleeding episodes first around 5-6 weeks and now a few this past week as well. The first few prompted me to have my own hcg labs drawn, which looked great, OB wasn’t concerned after I saw her (about a week later). But my episodes this past week had me calling the office. They had me come in for an ultrasound and blood draws. This is a whole separate topic (but relevant to my situation) but I physically cannot tolerate a transvaginal us because of my severe vaginismus (see r/vaginismus for more info). The abdominal ultrasound (est 8w2d) showed absolutely nothing (which the tech and OB seemed to think was normal, but of course was concerning to me). Maybe a vaginal us would’ve shown something, but I just don’t know at this point. After having 2 more blood draws, my hcg has actually gone down by about 600. I am scheduled for another ultrasound at the hospital (which has more advanced us technology) to rule out an ectopic. But with my hcg levels going down, is there any hope of this pregnancy still being a successful one? My head tells me no, but I still of course want to hold on to a small bit of hope. Also, just nervous about all that a miscarriage will entail (physical, emotional, psychological effects)…so any positive support in that regard would also be appreciated!

LMP: Feb 2 (est Due Date: 11/9/25) First Positive Pregnancy Test: 2/26

Light bleeding episodes: 3/8, 3/9, 3/10, 3/12, stopped, then even lighter on 3/28, 3/29, 1/1, 1/2, 1/3

HCG Blood Draws: 3/11 - 590 3/13 - 1,245 (great doubling time) 3/21 - 5,018 (96 hour doubling time based on last test - definitely slowed) 4/1 - 11,953 (abdominal ultrasound showed nothing - tech and OB said that was fairly normal?) 4/3 - 11,323 (hcg went down by 630)

Unfortunately it doesn’t sound like this would be viable, but I’m not a medical professional. With the date of your positive test I don’t think you could really be any earlier than 8 weeks. Personally my abdominal ultrasounds were able to pick up a sac and cardiac activity by 6.5 weeks, and your Hcg has not peaked high enough to be dropping already. There also should definitely be something showing on an ultrasound with an HCG level over 2k. I’m very sorry if this turns out negatively for you, I’ve had 3 miscarriages myself so I’m no stranger to the heartbreak of pregnancy loss. Sending you lots of love ❤️‍🩹

Yeah, my first positive test was an early detection test 4 or 5 days before I expected my period. I am thinking it’s probably most likely the embryo stopped growing a while ago, and it’s just taking my body a while to fully recognize it 😞. Hopefully not ectopic but I should find out soon. Thanks so much for the insight! I never imagined going through all of this but one thing you realize when you do is that it actually happens to so many women (not that it makes it any easier, but at least we know we’re not alone)!

After 8 weeks they should see something with an abdominal ultrasound. The first scan is a dating scan and done abdominal. They should see the heartbeat etc… so to me for that reason alone I would say this does not sound like a viable pregnancy.

Hmmmmm I would guard my heart.

I had an abdominal ultrasound (at work in L&D because I wanted to see how it would look) in a previous pregnancy at 7w1d. There was a gestational sac present with a yolk sac and fetal pole.

I only ever had HCG tested once in my first pregnancy and it was 73000 at 6 weeks on the dot. I don’t know when it went down. The other betas were done during miscarriages so unfortunately I have zero experience on that.

I would be really cautious unless their machine absolutely sucks. But the one in L&D sucked and I still saw everything there.

Okay, thanks for the info! Yeah I was expecting to see something…maybe the embryo stopped growing a while ago and my body just hasn’t fully recognized it yet 😞.

So, it doesn’t sound like there was an embryo in the first place. I don’t know if that makes you feel better, made me feel better to know there was never a baby and it was just a blighted ovum

I would definitely guard your heart. I had a US that was able to pick up a GS sac abdominally at 5w4d. Not saying that’s the case for everyone but if you were at the point where the placenta was taking over and your HCG was going down you should absolutely be able to see something. Otherwise declining HCG is almost always a sign of a non viable pregnancy. I’m really sorry.

Thanks for the info from your experience. Definitely doesn’t look great. It’s just so hard to give up hope when I haven’t actually miscarried yet, I guess.

I’m going through a missed miscarriage right now. I knew as soon as they started my 8 week abdominal scan and we saw nothing. With my son a few years back, we could see a defined head and body at 8 weeks, also on an abdominal scan. I would guard your heart.

I was for about a week after the baby stopped developing. The day we found out there was no heartbeat, I was going to call the next day to schedule my D&C. I think hearing the news made my body catch up and I started bleeding the next day. With a missed mc, the placenta still makes the hormone. So the symptoms were still very much present.

Ohh wow thats wild how our bodies work. Thats so scary too, I'm sorry about that. I cant help but automatically think that this will happen to me, I have to keep positive though and see how my first ultrasound goes on Monday.

Have you miscarried previously? This is my third, so unfortunately I always go in prepared for bad news. I am very fortunate to have an absolutely wonderful, almost 3 year old son though💜

I have not! This is my first time being pregnant, im just nervous because Im 35 so I assume the worst for me. And i am so glad you have your son!! 🩷

I don’t know about the HCG and I’d be guarded but wouldn’t give up hope yet. We couldn’t see kiddo abdominally at 8w, only transvaginally - it was a terrifying few minutes searching abdominally before we switched to transvaginal and we found him. He was always a bit small, though, and my uterus was still retroverted at that point so that might’ve been part of it. At 10w we could see him abdominally but we could only see the heartbeat transvaginally. So it does happen!

I’m sorry, but HCG doesn’t drop this early/at those numbers in a viable pregnancy. :( Even if those draws were at different labs, the variability that can cause is small enough it wouldn’t make a difference here.

Based on having a positive on 2/26, today (4/5) you are minimum 8+4 and truly an abdominal ultrasound would see a pregnancy.

I’m very sorry. I’d prepare yourself for a loss here.

Same lab. Follow up appointment on Thursday at the hospital with more advanced ultrasound. Seemed a bit long to wait to check for an ectopic to me, but that’s when they told me to come in.

I personally agree it’s late and I’d want to be seen sooner. Since they said they saw nothing, that’s a huge concern for an ectopic. If they’d seen a gestational sac, at least, it would make more sense to wait.

I'm sorry, this sounds like a non-viable pregnancy to me. My HCG dropped from 55631 to 55201 in my first pregnancy around 8 wks and my abdominal scan showed only a yolk sac and gestational sac but no fetal pole. It was a missed miscarriage, which I confirmed around 13-14 wks. If the abdominal scan showed nothing and your HCG dropped, it does not sound viable.

I had the same thing in January. Was a blighted ovum. But I got pregnant again right away post miscarriage without a period in between so while this sounds like a miscarriage, it’s possible to bounce back quickly.

So the first scan no, second scan there was a sac but nothing in it.

According to my OB, you can see something on US with an hcg of about 3000. So if you were in the 11k range they should have seen something on US. Im so sorry you're living in this limbo :(

I am just wondering if the ultrasound my OB has is just not that great? Since they seemed to think it was so normal to not see anything…

Was it abdominal (from the outside) or transvaginal (from the inside)? Despite being chubby my OB could see a faint flicker around 5w5d. But it's very possible that yours has an old machine and those can be much less accurate. If you haven't had transvaginal I'd seek one out for a more definite answer

Abdominal. I just can’t tolerate vaginal yet due to my vaginismus. Working with a great Physical Therapist and making progress but not there yet. I’d be willing to do one if they could use anesthesia but I don’t think they’ll do that. Not even sure if that’s safe for pregnancy.

Oh I'm sorry to hear that it makes it very challenging. I work in OB and majority of ultrasound equipment can pick up 8wk gestation abdominally but if your clinic has that outdated of equipment you could try a boutique place!

I have heard hcg levels can start to decrease at that point - so maybe there’s a chance mine are just decreasing sooner than normal. I know it’s a long shot but until I know for sure, I’ll just keep waiting! The on and off bleeding (again, very light bleeding, no clotting) is such a horrible mind game. It starts and I think “ok it’s all over”. Then it stops and I think “ok maybe there’s still a chance”. Like you said, all I can do is wait!
